There is no single meaningful price for an ED injection without knowing the prescription and what the quote includes. A package of alprostadil syringes and a vial of compounded Trimix are different purchases. Even two Trimix quotes can describe different concentrations, volumes, and usable supplies. Compare the complete treatment plan after the clinician has determined what to prescribe.

Caverject price, Caverject Impulse cost, and Trimix price are different quotes

Compare the full prescribed supply and care costs, not just a vial or syringe price. Caverject and Caverject Impulse contain alprostadil, while Trimix is a compounded combination. A pharmacy quote for one presentation cannot establish the cost of the other. A clinic package may also include teaching or follow-up that a medication-only quote leaves out.

Asking for those details produces a comparison you can act on; presenting an undated average as today's price does not.

A dual-chamber syringe is not two usable injections

Pfizer's Caverject Impulse instructions describe a dual-chamber system used to prepare the medication and select the prescribed amount. The chambers do not represent two patient doses. Follow the single-use and discard instructions rather than counting leftover solution as another injection when calculating cost.

That distinction can change a comparison even when two packages display the same number of micrograms. Ask the pharmacist how many prescribed uses the package actually provides. Do not convert micrograms, milliliters, or syringe markings from one product into another on the basis of a price spreadsheet. A prescription with another concentration needs its own instructions.

Calculate the usable cost of a Trimix vial

The MSK patient guide explains that Trimix contains multiple ingredients and that storage matters. The dispensing pharmacy's exact concentration, beyond-use date, and storage instructions belong in the quote. A larger vial is not automatically better value if part of it must be discarded before it can be used as prescribed.

These numbers illustrate the calculation only; they are not pharmacy prices, vial sizes, dose recommendations, or claims about product stability. Use the pharmacist-confirmed usable supply in the denominator. Keep the first-period cost separate from later refills if the initial teaching visit will not recur with every order.

Questions for insurance and cash-pay comparisons

• Check medication coverage separately from office visits, teaching, supplies, and follow-up.
• Ask whether prior authorization, a particular pharmacy, or a quantity limit affects the quoted benefit.
• For a cash quote, confirm delivery, handling, needles, syringes, and any other required supplies.
• Ask when the next order is charged and whether automatic refills can be paused before processing.

A discount shown online is not an insurer's coverage decision, and a pharmacy's stock status is not a promise that the prescriber considers that product appropriate. Record the date and the conditions attached to each quote. If an insurer or pharmacy cannot give a final price until the prescription is reviewed, keep that figure pending rather than replacing it with the cheapest advertised number.

Cost savings should not change the injection instructions

Memorial Sloan Kettering's patient guide describes Trimix as a combination of alprostadil, phentolamine, and papaverine. The name Trimix alone does not tell you the concentration or your prescribed injection volume.

Ask for the expected usable supply in plain language. A syringe marking describes a measured volume, not a universal strength shared by every compounded formula. Do not carry a volume from an old prescription over to a new concentration without explicit instructions.

Budget for teaching and supervised dose selection

Caverject Impulse's label places the first doses in the healthcare provider's office and requires instruction and assessment before home use. MSK describes a program that generally uses two teaching visits. That is one center's process, not a promise that every clinic uses the same number of visits or charges the same fee.

• Ask whether the initial appointment, test medication, and teaching are billed separately.
• Check the self-pay quote or expected insurance responsibility for each visit.
• Ask what follow-up is included while the prescribed dose is being established.
• Clarify who answers questions between visits and outside normal office hours.

Storage can change the value of a larger order

MSK notes that the alprostadil in Trimix can weaken over time and discusses refrigeration and protection from light. Follow the dispensing pharmacy's instructions for your actual formulation, including shipping, storage, and the beyond-use or discard date. Do not assume every compounded vial or commercial alprostadil presentation shares the same storage rules.

Before paying for several months at once, ask how that supply fits the prescription and storage limits. Clarify what happens if the dose changes during follow-up or a shipment arrives outside its required conditions. A lower price for medication you cannot use does not lower your treatment cost.
